Appointments
To make an appointment, call our office at (713) 795-5160. Office appointments are available daily, and patients are accommodated quickly. Patients are seen by physician referral or self referral. In addition to our Texas Medical Center office, we have offices in The Woodlands, Kingwood, and Sugar Land. We have a bilingual staff and our office environment is friendly to children.
Prescriptions
We prefer to refill your medications at the time of your office visit. Please bring your medication list with you to each appointment. We require 48 hours notice of routine refill requests. Please be aware that most requests for pain medications and antibiotics require an appointment. We require at least yearly visits for stable medication problems, but more frequent visits may be required as determined by your provider.
No refills will be given after hours, weekends or holidays.
Release of Records
A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, phone number, and fax number.

What Do We Need From You?
- New Patients: Please print and fill out our on-line registration forms, and bring them with you when you come in for your appointment.
- Please bring all of the medications you are taking, in the pharmacy container, when you come for your appointment.
- Please let us know of any pertinent changes in insurance, employment, change of address, or relationships with other care/service givers. Please see our financial policy for more details.
- Please arrive 15 minutes early if possible. Please arrive on time for scheduled appointments and cancel, when necessary, with a phone call.
